First of all: I like the technique. The light and shadow effects are good and the tongues looks really wet (I guess with drops of saliva dripping from them it would look much more scary). I also like the colors, everything is tone in tone. You created an interesting cotnrast with this: The colors blend in with each other while the depicted head is really creepy.
Suprising for me was that though it is a really creepy head it does not look too monstrous to me but has more of a surprised look on it. I guess that comes from the opened ("real") mouth since it looks like a normal human mouth with pinted teeth but not monstrous big with fangs reaching to the chin.
What I really like the fact that all senses (seeing, hearing, smelling) are turned into something carnivorous (tongues come out of ears and eyes) or aren't there (no nose). Even the brain where you think and plan is substituted by something carnivorous.
I'm sorry for the bad english; I haven't written english for quite some time now. All in all I want to say: Great picture, keep going
Best not do a google image search for 'trypophobia skin' then. But it's already too late; just like an accident scene, morbid curiosity compels you to look
